What you may be dealing with
We ask when symptoms start, whether they travel into a leg, and what changes them. Lifting, prolonged sitting and rotation may require different starting points for exercise and activity.
Deadlifts, picking up kids or moving boxes bring on a tight, achy back.
Long drives and desk days leave you stiff when you stand up.
Golf swings, yard work or turning to reach something catches or aches.
Pain, tingling or numbness traveling into the buttock or leg.
Episodes that come and go, and a fear of doing something that will set it off.
Conditions we help with
Most episodes of back pain are not caused by one precise structure, and the evaluation focuses on what changes your symptoms.
New loss of bowel or bladder control, numbness around the groin or saddle area, or rapidly worsening leg weakness needs emergency assessment. Severe unexplained pain or symptoms with fever, major trauma or other concerning changes need prompt medical assessment rather than routine physical therapy.
Our approach
The goal is to find positions and loads that your back tolerates, and then expand them.
What provokes it, what eases it, what you have tried, and what you are afraid to do.
Spine, hip and shoulder mobility, trunk and leg strength, and how you hinge and rotate.
Hands-on care, dry needling if suitable, and honest explanation of what your findings mean.
Progressive loading so the back is better prepared for what you do, including lifting.
Why Fulcrum
We adjust range, resistance and volume to build toward the lifting your work or training requires.
Neurodynamics training informs the assessment when back symptoms travel into the leg.

Return to activity
Your goals might involve lifting at work, finishing a round of golf or sitting through a commute. Progress is assessed against those specific tasks.
Your plan builds toward the activities you choose, with progress reviewed along the way.

Good to know
Prolonged rest is not usually helpful for most back pain, but what you should do depends on your situation. Your evaluation will outline what to keep doing.
Often, with changes. We work out which lifts, loads and positions are tolerable and use them as the starting point for progress.
You do not need imaging to be evaluated. Findings on imaging are common even in people without pain. If your exam suggests you need a physician or imaging, we will recommend it.
It can help many people manage leg symptoms and rebuild activity tolerance. Your evaluation checks nerve-related symptoms and whether medical follow-up is needed.
It can be considered when muscle guarding is a feature, always alongside exercise rather than alone.
You can usually start with an evaluation without a referral under Florida's direct-access rules. Continuing care may require a referral, and insurance requirements vary; ask us about care requirements and your insurer about coverage.
We offer out-of-network and self-pay care, accept HSA/FSA cards, and bill Medicare directly. See our Insurance and Payment page for rates, superbills and plan-dependent reimbursement details.
Your evaluation and follow-up visits are full 60-minute, one-on-one sessions with the same physical therapist, with time for treatment, exercise and questions.
