When muscle pain is limiting movement

People usually ask about dry needling when a muscle feels knotted, tender or guarded and stretching, massage or rolling only help for a short while.

01 /

Persistent tight spots

Shoulders, neck, hips, calves or low back that feel clenched no matter what you do.

02 /

Pain that limits training

Stiffness or soreness that cuts your range or keeps you from loading well.

03 /

Tendon and muscle overuse

Aching that follows volume increases in lifting, running or racquet sports.

04 /

Questions before trying it

You want to know what the needles feel like and whether the technique fits your symptoms.

Conditions we help with

Where dry needling may be considered

Your exam helps determine whether dry needling could support movement and exercise in these areas.

Evidence varies by condition, and relief may be short-term. Exercise and progressive loading remain central to your plan.

  • Muscle tension around the neck and shoulder
  • Low back and hip muscle guarding
  • Calf, hamstring and quad tightness
  • Some overuse tendon and muscle complaints
  • Muscle pain that limits exercise progress

Our approach

How dry needling fits into care

A thin, solid filament needle is placed into muscle or soft tissue. No medication is injected. It is combined with the rest of your plan.

  1. 01

    Evaluate first

We assess your symptoms, health history and movement before deciding whether dry needling is appropriate.

  2. 02

    Discuss and consent

We discuss the expected sensations, potential benefits and risks before treatment. You can decline.

  3. 03

    Pair with movement

Treatment is followed by exercise and movement work to use any change in comfort or motion.

  4. 04

    Reassess

We check what changed and keep or drop it from your plan based on your response.

  • Sitting, driving and working with less tension

Good to know

Common questions.

What is dry needling?

A physical therapy technique using a thin filament needle in muscle or soft tissue to address painful or tight areas. It is called dry because nothing is injected.

Is it the same as acupuncture?

No. They use similar needles but differ in their approach and rationale. Dry needling in physical therapy is directed by a musculoskeletal evaluation.

Does it hurt?

Many people feel little at insertion. You may feel a brief ache, twitch or cramp in the muscle, and some soreness afterward. Tell us what you feel so we can adjust.

Who should not have dry needling?

Tell your physical therapist about bleeding disorders, blood-thinning medication, pregnancy, infection or previous reactions to needles. We review your history and the treatment area before deciding whether it is appropriate.

Is dry needling a stand-alone treatment?

At Fulcrum, it is offered within a physical therapy visit. Your plan also addresses the strength and activity demands behind your goals.

Is dry needling covered by Medicare or insurance?

Ask us about billing before your visit. Coverage depends on your plan and, for Medicare, Medicare's rules. The Insurance page explains our payment model.

Do I need a referral?

You can usually start with an evaluation without a referral under Florida's direct-access rules. Continuing care may require a referral, and insurance requirements vary; ask us about care requirements and your insurer about coverage.

How does payment work, and do you accept insurance?

We offer out-of-network and self-pay care, accept HSA/FSA cards, and bill Medicare directly. See our Insurance and Payment page for rates, superbills and plan-dependent reimbursement details.
