This policy describes how the Fulcrum Physical Therapy website handles information. The website is operated by Fulcrum Physical Therapy LLC, a Florida, United States entity, at 3973 Eagle Landing Parkway, Orange Park, FL 32065. This policy is written with Florida and United States practice in mind and does not claim compliance with the laws of every jurisdiction.

Contact form

Name, email, phone, visit type, reason for visit, preferred day and time, how you heard about us, and the referring service or condition page, when included. The request is sent through our backend and the Resend email service to the clinic's email. The confirmation email sent to you includes the details you submitted.

Event registration form

Name, email, optional phone, and the time slot you choose. This is stored in a PostgreSQL database and the clinic is notified by email.

Technical information

Basic request, device, IP address, and browser data may be processed by our hosting and service providers to operate and secure the site.

Submitting a form is not consent to marketing messages.

• Replit: website hosting and connected database infrastructure.
• Resend: transactional email delivery for form notifications and confirmations.
• Clinic email provider: receives requests in the clinic inbox.
• Google Fonts: fonts are loaded from Google's servers.
• Unsplash: where an external image from Unsplash is used, it loads from their servers.
• Google Maps and Google Reviews: links to these services are external and subject to Google's own policies.

When fonts or images load from external services, those services receive technical request information such as your IP address and browser details. This happens whether or not you click any external link.

Where Google Analytics is enabled, the site's event reporting can include the page, the action, and the visit type. This event reporting does not include form names, emails, phone numbers, or reasons for visit.

The site's own application code does not set cookies or use browser storage. Hosting and external services may use their own technologies. You can manage cookies in your browser settings, though doing so may affect some features. We do not offer a separate consent manager or opt-out tool.

We share information with the providers above as needed to deliver your requests and operate the site, and where required by law.

Retention of information in the clinic inbox, event database, and provider systems depends on its purpose and applicable record-keeping requirements. Contact us with questions about retention of your information. Legal clinical-record requirements may limit deletion.

No method of transmission or storage is perfectly secure, and we cannot guarantee absolute security.

To ask about access, correction, or deletion, or to ask a question, email hussein@fulcrumphysicaltherapy.com. We may need to verify your identity. Legal record-keeping duties may limit what can be deleted.

Our forms are not designed for children under 13 to use on their own. A parent or guardian should contact the clinic on behalf of a child. If a child's information was submitted, please contact us.

This website policy is not a HIPAA Notice of Privacy Practices. Please do not submit detailed medical history, diagnoses, insurance ID numbers, or Social Security numbers through web forms. The website is not a patient portal and is not for emergency care. In an emergency, call 911.
