Insurance and Cash Pay

Use Your Benefits or Choose Cash Pay

Fulcrum offers straightforward cash pay and works with eligible out of network benefits from many major insurance plans. If you use your benefits, we submit your superbills on your behalf so you do not have to manage the claim paperwork.

How It Works

Choose simple cash pay or use eligible out of network reimbursement. We handle superbill submission for you, and you know what to expect before your first visit.

01

Schedule your visit

Call us or request an appointment online. You can usually start with an evaluation without a referral.

02

Receive your full session

60 minutes, one-on-one with Dr. Eid, every time.

03

Pay at your visit

Choose cash pay, credit, debit, HSA, or FSA.

04

We submit your superbill

We submit the claim to your insurer on your behalf, so you do not have to manage the paperwork.

Why This Model?

Most physical therapy clinics see 4 to 5 patients per hour to meet insurance reimbursement requirements. We see one.

Every session is 60 minutes with one doctor and one patient. There are no aides, no handoffs, and no watching the clock. That is only possible outside the insurance volume model.

Without insurance restrictions, Dr. Eid is free to spend the time your case actually requires, use the techniques that work best for you, and adjust your plan visit by visit.

60-minute sessions, every visit, with no exceptions

Always Dr. Eid, never an aide or technician

Coverage questions can be discussed before starting care

An individualized plan, not a preset visit package

Treatment tailored to you, not to billing codes

The Real Cost Comparison

Your total cost depends on your plan, deductible, visit frequency and any out-of-network reimbursement. Compare the actual benefits in your policy rather than assuming one model always costs less.

Frequently Asked Questions

Straight answers to the questions we hear most.

What is a superbill?

A superbill is a detailed receipt containing the medical codes your insurance company needs to process an out of network claim. We prepare and submit it to your insurer on your behalf after each visit.

Do I need a referral?

You can usually start with a physical therapy evaluation without a physician referral in Florida. Requirements for continued treatment and insurance coverage can differ; we can discuss what applies to your visit.

What if I have an HMO?

Many HMO plans do not include out-of-network benefits. Ask your insurer whether your specific plan covers out-of-network physical therapy and what requirements apply. Self-pay options are also available.

How do I submit my claim?

You do not need to submit it yourself. Fulcrum prepares and submits your superbill to your insurance company on your behalf. Reimbursement amounts and processing times depend on your individual plan.

How long until I get reimbursed?

Processing times depend on your insurer and plan. Contact your insurer for an estimate; reimbursement is not guaranteed.

What if my claim is denied?

We can help clarify the claim documentation, but your insurer makes the coverage decision. Ask about your out-of-network benefits before scheduling; a correctly submitted claim does not guarantee reimbursement.

Can I use my HSA or FSA card?

We accept HSA and FSA cards. Check your account rules and eligible-expense requirements, and retain your receipt.

Can I pay cash without using insurance?

Our cash-pay initial evaluation is $175 and follow-up treatment is $125. Medicare-covered services follow Medicare billing rules and beneficiary responsibility rather than these cash-pay rates. Contact us with payment questions before scheduling.

Why are you out-of-network?

Fulcrum's out-of-network model supports individualized, full 60-minute sessions with Dr. Eid. We plan visits around your clinical needs and activity goals. Medicare remains a directly billed exception.

Ready to Get Started?

Ask your insurer about your out-of-network physical therapy benefits. Call us with questions about payment or superbill documentation, or request an evaluation online.
